SERVICE ORDER AGREEMENT

This Service Order Agreement (“Agreement”) is effective as of the date of last signature on this Order (“Effective Date”) between Prolink Healthcare, LLC, an Ohio limited liability company (“Prolink”) and Choose an item., a(n) Choose an item. Choose an item., (“Client”). 

1. Scope of Services

By checking one or more boxes below, Client acknowledges and agrees that each selected Service is a contracted Service under this Agreement and subject to the applicable terms described below. “Services” means the specific items selected by Client from the list below. Client may not select both Master Supplier Services and Staffing Services; if both are selected, it shall be conclusively presumed that, as between those two, Master Supplier Services is the Service being requested, and any provisions applicable to Staffing Services shall not apply.

2. Incorporation by Reference

Each party acknowledges and agrees that the Master Terms and Conditions available at www.prolinkworks.com/mastertermsandconditions_12-1-2025 (the “Master Terms and Conditions” or “Terms”) are hereby incorporated into this Agreement by reference as if fully set forth herein, and that the Services to be provided by Prolink are incorporated by reference in the Master Terms and Conditions. Each signatory represents and warrants that they have full power and legal authority to bind their respective party to this Agreement. Intending to be legally bound, the parties have entered this Agreement as of the Effective Date.
