Responsibilities:
- Utilization management, case management, and care coordination management of his/her patients from the time of diagnosis to 90 days post-discharge.
- Connects with patient at the time of diagnosis  and follows the patient across the hospital stay and 90 days post discharge. G
- Coordinate discharge planning.
-Ensured quality cost-effective patient outcomes. 


Shift/Hours: 
FT, 40 hrs/wk, Monday-Friday, 8:30am-5pm

Compensation
Pay: Starts at $71,900
Sign On Bonus: $10,000 for 1 year Case Management experience (any setting) OR 2 years of critical care experience (ICU/Stepdown)


Requirements
License: Current professional Nursing Licensure in the state of IL required    
Certification: Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross).      
Experience:  A minimum of 1 year of Care/Case management experience in the acute setting with Utilization Management Experience, and a minimum of 2 years clinical nursing experience required in any area.  Very firm on experience requirement.     
Education: BSN      


Preferred:    
Education: Bachelor's degree in Nursing from a NLN (National League for Nursing) accredited school of nursing is preferred.    
Certification: Clinical certification, such as case management certification, is beneficial.     
Experience: Previous experience in Utilization Management, Managed Care or chronic disease management highly preferred. Experience with Electronic Medical Record (EMR) platform preferred. Knowledge of InterQual or MCG criteria is preferred.    
Other: Knowledge of community resources    


About the Unit:
Assigned Unit:    Continuum Care Manager may work in any unit in the hospital. The Managers try to maintain consistency on the weekdays. They may go to different units on the weekend, or cover for a call-off. There have been individuals on the same unit for years. They are usually assigned to one to two units regularly. 
Patient Population and Case Load:    Adults. Current case load is 18 patients/day on average. It can range from 16-20.
What type of personality works well in this department?    Someone adaptable. Someone who can connect with patients and families in a short period of time. They can multitask. Not easily flustered. Confident. Good verbal skills.
